Qualified Participants:

• Adults ages 18 and older
• Has had a diagnosis of CD at least 3 months (confirmed by
  ileocolonoscopy + histology). For participants with no documented confirmation of CD
  diagnosis or if previous diagnosis is not deemed conclusive, CD diagnosis must be
  confirmed (by local pathology report).
• Have moderate to Severe Crohn’s Disease
